Medication Management

Poor medication management is one of the main causes of patients being readmitted to the hospital after initial discharge. Taking the wrong dosage, not understanding how to properly take medications, improperly stopping medications, and mixing incompatible prescription and over-the-counter drugs—these are just a few common mistakes that can put your health or the health of a loved one in jeopardy.

The Christiana Care Visiting Nurse Association can help you to prevent this common but dangerous problem. Here's how we can help:

  • Medication audit: Our nurses can help you or a loved one identify and reconcile the medications prescribed and consumed.
  • Pain management: We can work with your physicians to calibrate your pain medication so that you are comfortable.
  • Medication education: Our nurses can teach you or your caregiver what medications you are taking, why you are taking them and what side-effects you should look for.
