Home Rehabilitative Services

The Visiting Nurse Association's rehabilitative team can provide you with the full range of rehabilitative services that you need to recover at home. Your very first visit from our rehab team will include a thorough evaluation of your home and your functional ability. Based on this assessment and under the direction of your doctor, we will establish an individual care plan that will set you on your optimal road to recovery.

Our rehabilitative team includes physical therapists and physical-therapist assistants, occupational therapists and certified occupational-therapy assistants, and a speech-language pathologist. Each of these highly trained, experienced therapists provides an essential componenet to your total rehabilitative care:

Home health physical therapist

Physical therapists (PTs) are experts in movement. Home-health PTs are especially knowledgeable of movements needed to maximize a patient's ability to be safe at home. Your home physical therapist will assess your strength, range of motion, balance, endurance and functional movements such as ambulation and transfers within the home. Your physical therapist will also assess your living quarters to minimize safety hazards and reduce the risk of falls.

Home-health physical therapists are commonly called upon to assist people recovering from:

  • Hip or knee replacements.
  • Orthopaedic injuries, including fractures or trauma.
  • Neurological disorders such as stroke, Parkinson's disease, multiple sclerosis, and head or spinal injuries.
  • Heart failure.
  • Pulmonary conditions (COPD).

Physical therapy treatments focus on:

  • Gait and balance.
  • Transfers and functional mobility.
  • Equipment management.
  • Fall-prevention strategies.
  • Pain management.
  • Therapeutic exercise.

Home health occupational therapist

Occupational Therapists (OTs) are experts in movement. Home-health OTs are especially knowledgeable about movements needed to maximize activities of daily living. Your occupational therapist will assess your ability to perform self-care tasks such as bathing, grooming and dressing. Your occupational therapist will also assess your need for adaptive and safety equipment needed throughout the home.

Home-health occupational therapists are commonly called on to assist people who are recovering from:

  • Hip or knee replacement.
  • Orthopaedic injury, including fracture or trauma.
  • Neurological disorders such as stroke, Parkinson's disease, multiple sclerosis, and head or spinal injuries.
  • Cardio-pulmonary conditions.
  • Visual and cognitive deficits.

Occupational therapy treatments focus on:

  • Training in the skills needed to perform activities of daily living.
  • Transfers and functional mobility.
  • Equipment management.
  • Conserving your body's energy.
  • Fall-prevention strategies.
  • Pain management.
  • Therapeutic exercise.

Home health speech therapist

Speech language pathologists (SLPs), also referred to as speech therapists, are experts in evaluating and treating swallowing and communication disorders, as well as cognitive impairments. Home-health speech therapists are commonly called upon to assist people with neurological disorders, swallowing and voice disorders, and cognitive and language disorders.

Speech-therapy treatment focuses on:

  • Communication strategies.
  • Speech or voice retraining.
  • Swallowing techniques.
  • Cognitive management for conditions such as dementia.
