Chronic Disease Management

You or your loved one can remain independent and healthy at home, and the Christiana Care VNA can help. Our nurses, aides and therapists can show you the best ways to care for your chronic disease in the comfort of your own home. And because we keep your doctor updated on your progress, you will gain added peace of mind that your health care team is working in a coordinated manner to ensure your best health.

Using nationally-recognized standards of care and evidence-based medicine, we care for a wide variety chronic conditions at home:

Heart failure

Our licensed cardiac specialty nurses help patients recovering from heart attacks and heart bypass surgery. We assist patients who are learning to manage heart failure or addressing other cardiac concerns. Our TeleHealth program, a telephone-based monitoring system, regularly updates our nurses and your doctors.

Cancer

Our Living with Cancer program provides nurses who help with pain and symptom management, rehabilitation, nutrition, and physical, occupational and speech therapies. Our skilled nurses manage chemotherapy and infusion therapies in the home, including managing ports and access devices. Our patient advocates work with insurance companies on our patients' behalf and provide access to available social services. We work as part of the team at the Helen F. Graham Cancer Center.

Stroke

Our skilled nursing and rehabilitation teams help stroke victims and their caregivers manage the transition from hospital to home after stroke. Nurses, aides, therapists and support personnel give patients and caregivers the skills to recover lost ability. They also provide education about the warning signs and risk factors for stroke. We treat recovering stroke patients (plus anyone who has a stroke-related disorder such as weakness, paralysis, or dysphagia) and empower them to take control of their health.

Diabetes

Living with diabetes is easier with coaching and educational materials from our nurses and therapists. They teach the patient and family how to adopt behaviors that will make leaving with diabetes easier. Using guidelines from the American Diabetes Association, our nurses work with diabetic patients to regularly monitory blood sugar, recognize the complications associated with diabetes and seek medical attention when necessary.

 

Chronic obstructive pulmonary disease (COPD)

Our highly-respected home-care program for patients with chronic obstructive pulmonary disease teaches patients and their caregivers to maintain their lung health and recognize risks for rehospitalization. Patients and their personal caregivers are actively involved in managing care. We use telehealth, a remote monitoring program that regularly updates our nurses and doctors.
